Listen with the door switch held in
The fan runs with the door shut, so the test is to hold the switch and listen. A fan that only chirps when cold is still a failing fan.

No air at the fresh-food outlet with a cold freezer is the classic reading for this.

Look for ice on the blade and the shroud
Where the blade is packed in ice, the fan is a symptom and the defrost circuit is the cause. Replacing the motor alone guarantees a return visit.

The fan is loud and the refrigerator still cools. Can it wait?
It can, but not indefinitely, and the noise is the cheap part of the warning. A motor grinding in its bearings is drawing more current than it should and slowing down, and the temperature in the fresh-food section starts drifting up long before the fan actually stops.
Why did the fan fail again a year later?
Usually because the reason it stalled the first time was ice, not age. If the defrost heater or its thermostat is dead, frost rebuilds on the coil and the new blade meets the same wall.
